Job Description

On behalf of our client, we are seeking an enthusiastic Discovery Coordinator / Community Connector to join our client's facility in Co. Dublin. Applicants must have third level education in social care or related fields and experience working with people with disabilities. The successful candidate will be required to demonstrate effective communication skills and positive risk management. You will build a relationship with the individual to support them to have the life of their choosing. They will also create opportunities for people with disabilities in their local community by developing good working relationships with community groups.
Responsibilities:
" To provide information, advice and guidance to the individual supported, support the individual to access services on matters relating to finance, good housekeeping, health & welfare, safety and security and daily work and leisure activities. Ensure that this is done in a manner that respects the individual's dignity, privacy and choice.
" To work in a way that reflects the values of Supported Self Directed Living.
" To establish steadfast links and build productive relationships in the local community.
" To promote a balance between expressed wishes and choices of the individual whilst ensuring the individual competency in relevant settings.
" To provide strategic support for making choices that will lead to a desirable life for the individual.
" To empower individuals and their families to speak up for their needs and desires.
" To advocate for the individual if necessary.
" To provide accurate information around choices and needs.
